DUTIES AND RESPONSIBILITIES:
Assist and provide physical therapy services in functional activities, modalities, and other muscular exercises to disabled children and youth.
Work with the Special Education Program and related agencies services providers, teachers, specialist to organize treatment plans for children and youth with disabilities.
Assist families in home assessment, functional treatment and make recommendations for services needed.
Use of modalities techniques, massage and other equipment to provide physical therapy services.
Conduct home visits and meetings with parents.
Prepare and submit monthly services reports, student’s quarterly progress reports and yearly reports.
Maintain, repair and prescribed assistive devices for the disable children and youth.
Conduct in service training on assistive devices and others.
Perform other duties as assigned by supervisor.
MINIMUM QUALIFICATIONS AND OTHER REQUIREMENTS:
High School Diploma with at least two (2) years of work related experience.
